This promotes good health and one on one patient care in the comfort of their own home.ROCHESTER REGIONAL HEALTH HOSPICE HOME CAREOur Hospice Home Care team provides hospice and palliative care services within the comfort of our patients' homes. Our Nursing and Therapy teams work directly with patients and support their families through the loss of a loved one. We partner with our Hildebrandt Hospice Care Center, our hospitals and health facilities to provide end to end continuity of patient care. We are a responsive, collaborative partner in advocating for, and providing access to effective, innovative, culturally competent and cost-efficient services for the communities we serve.STATUS: -Full Time - - - - - - - - - - - -LOCATION: -Monroe County - - - - -DEPARTMENT: -Home Care HospiceSCHEDULE: -WeekendsATTRIBUTES
    • Compassionate, independent ,and patient focused
    • Comprehensive nursing assessments and documentation, decisive judgment and professional behavior
    • Two (2) years experience in home care preferred
    • Bilingual preferred
    • At least one year of nursing experienceRESPONSIBILITIES
      • Patient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care
      • Planning. Develop and document individualized care plans customized for each patient's unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress
      • Communication. Maintain effective communication to convey patient health status, treatment plans and progress; communicate with office and supervisory staff regularly; coordinates client services and/or referrals in an efficient and timely manner
      • Documentation. Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards
      • Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirementsEDUCATION:AS: Nursing (Required)LICENSES / CERTIFICATIONS: -BLS - Basic Life Support - American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements. PAY RANGE:$73,528.00 - $98,072.00CITY:NewarkPOSTAL CODE:14513The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
